Phone Appointments:
Please note that phone/ video appointments are not on time. When booking a virtual appointment please expect a delay in the call.
If you missed your virtual appointment, we will need to reschedule during Dr. Fanaei’s next available date.
Dr. Fanaei does his best to get to all appointments on time.
New Patients:
If you would like to be on our waitlist, please call our office at (905)-597-4443.
Prescription:
For any new prescriptions you will need to book an appointment with Dr. Fanaei, please book an appointment online through our booking system or call the office at (905)-597-4443.
If you need a refill, please contact our office "ONE" week prior to your prescription finishing. You can schedule a phone appointment with Dr. Fanaei. Going forward, Dr. Fanaei will need to speak to the patient/ visit the patient directly and cannot accept the refill request from the pharmacy.
Our clinic policies
1. Bring your valid Health Card. All patients must provide a valid Provincial Health Card at the time of each visit. If you are visitor to Canada or you have refugee status, you are required to provide us with original valid photo ID
2. We schedule the last in-person visit thirty minutes prior to closing, the clinic will register the last in-person patient 30 minutes prior to closing time.
Prescription Renewals
We request that medication renewals be done during your regular appointment with your family doctor.
Please contact the clinic or schedule an online visit with your family doctor for medication renewals.
Health Cards
It is the patient’s responsibility to have a valid health card. There is a charge for appointments if no valid health card is
provided prior to the visit.
